Targeted Case Manager services (TCM) also known as Certified Behavioral Case Manager (CBHCM).
Services we provide
“Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote patient safety, quality of care, and cost-effective outcomes”
Coordinating psychiatric care by means of medical appointments, treatment, follow ups, and referrals to other specialist including a Primary Care Physician. Coordinating and collaborating with other disciplines appropriate referrals, obtaining pertinent medical records, and care coordination.
Linking individuals with community resources such as legal assistance, housing, substance abuse rehabilitation, transportation, federal, state, and local educational aid programs.
Assesing/Assisting individual’s needs (food, financial, employment, housing, medications, etc.) and assisting patients with the necessary aid to meet their needs.
Monitoring individual’s progress and update of treatment plan. TCM services is a continuous process that can be updated depending on the individual’s progress towards the set goals.
Planning meeting with patients for assessment, coordinating, monitoring, and planning services.
Planning, arranging, coordinating, obtaining, monitoring, or following up on specific aspect of treatment (e.g., medical Tx, substance dependence treatment, appointments with other providers).
Working with and collaborating with collateral contacts external to the agency including family members, landlords, and employers.
